TERMS AND CONDITIONS
REGULATIONS FOR THE USE OF THE IMPERIAL CLINIC OFFER
BY USING THE OFFER, YOU ACCEPT THESE REGULATIONS:

These regulations concern the purchase of treatments at the IMPERIAL CLINIC medical&aesthetic located in Warsaw, ul. Zygmunta Słomińskiego 7, as included in the Clinic's offer. Regular and promotional prices for treatments are available on the website www.imperialclinic.pl under the "CENNIK" (PRICE LIST) tab. If a promotion is active, its duration is specified for each treatment.

Treatments can be purchased at the reception of the Imperial Clinic medical&aesthetic at ul. Zygmunta Słomińskiego 7 in Warsaw, or by making a bank transfer to the company account and sending the payment confirmation to the email address: recepcja@imperialclinic.pl.

Treatments can only be performed at the Imperial Clinic medical&aesthetic located at ul. Zygmunta Słomińskiego 7 lok U3, 00-195 Warsaw.

To benefit from a promotion, treatments must be purchased during the promotional period, as indicated in the price list.

For bank transfers, the date of the transfer visible on the payment confirmation is binding for eligibility for the offer.

The person who has purchased a treatment is required to book an appointment at the Imperial Clinic medical&aesthetic in advance.

Purchased treatments are non-refundable and cannot be exchanged for cash. The Imperial Clinic medical&aesthetic allows for the exchange of purchased treatments for other treatments from the Imperial Clinic medical&aesthetic offer, for a gift voucher, or to be transferred to another person. However, please note that for any exchange, the current prices from the Imperial Clinic medical&aesthetic Price List will be considered. If exchanging for a more expensive treatment, the patient must pay the difference. If exchanging for a less expensive treatment, the Imperial Clinic medical&aesthetic will not refund the price difference.

Treatments purchased at the Imperial Clinic medical&aesthetic have no time limit, and the patient can use them at any time.

The Imperial Clinic medical&aesthetic reserves the right to modify any promotion, as well as to extend or shorten the duration of a promotion.

The Imperial Clinic medical&aesthetic reserves the right to cancel a promotion at any time, using the same form in which it was announced. The cancellation of a promotion is ineffective for those who have already participated in it, and they may use their purchased treatments according to the promotional terms.

When booking a first appointment lasting at least 1.5 hours, or if a client has previously failed to show up for an appointment without prior cancellation, regardless of the booking method, the patient may be asked to pay a deposit of 200 PLN (depending on the duration of the reserved treatment). This deposit will be deducted from the total cost of the treatment performed. The deposit must be paid within 48 hours of booking, either in person at the Clinic or via bank transfer to account number: 19 1020 1185 0000 4002 0338 0672. If the appointment is rescheduled with at least 24 hours' notice, the deposit will not be forfeited.

The Imperial Clinic medical&aesthetic will send a text message reminder to the patient 24 hours before the scheduled appointment.

If an appointment is not canceled at least 24 hours before its scheduled time:

The deposit is not returned to the client.

An appointment that is part of a package will be considered to have been used.

Before a treatment, the patient will be asked to complete a Patient Card. Failure to provide information on the Patient Card and a lack of signature on the consent form for the treatment will automatically result in the refusal to perform the service.

If there is a high risk of adverse reactions or the treatment is deemed unjustified, the technician has the right to refuse to perform the service.

Treatment for a minor patient requires the presence and consent of a legal guardian.

Patients' personal data will be processed for the purpose of providing services.

The purchase of treatments is equivalent to accepting the terms of these Regulations.
